Parent's GNIB card. Parent's travel document. Proof of citizenship of Irish adoptive parent, for example: original passport or original birth certificate. Temporary permissions granted to parents while applying for asylum are non-reckonable.

Periods of unlawful residence of parents are non-reckonable. See our information about making an observation request. If you want, we can include your previous passport number as an observation.

In the vast majority of cases it is not necessary to include an observation on your passport. It is not possible to have observations noted on passport cards. All Applications We accept internet printouts that are stamped by issuing authority. Certified translations are required if documents are not in English or Irish. Proof of name and proof of address cannot be covered by the same document. Some acceptance facilities require appointments and others do not. We recommend calling the acceptance facility ahead of time to verify their hours, acceptable forms of payment, and appointment requirements.

You can mail in your Form DS Additionally, the Department of State does not currently have the option to submit your passport application online. You can apply for both a passport book and a passport card at the same time on Form DS, but you must pay both fees.

I applied for both a passport book and passport card : You may receive three separate mailings; one with your citizenship documents, one with your newly-issued passport book, and one with your newly-issued passport card.

Please contact NPIC if you do not receive the second or third mailing within 4 weeks of the previous mailing. If you choose not to submit a photocopy, you may submit a second certified copy of your birth certificate. We will keep this second certified copy for our records.
